"Hillcrest Badminton Club". Forms accompanied with payment can be submitted

MEMBERSHIP AGREEMENT

Memberships are not transferable unless approved by the Club Executive.
No refunds will be permitted after the season starts.
Forms must be signed by the individual applying for membership. Unsigned
forms will NOT be accepted. Food and drink may not be taken into the
gymnasium. Only soft-soled, non-marking shoes used solely indoors may be worn.

The player/member signed above agrees to:

a) play at his or her own risk
b) accept responsibility for his or her acts and omissions
c) be wholly responsible for loss of or damage to personal property 
d) conduct himself/herself maturely and responsibly at all times or 
   lose the right to play, which may be revoked without notice or reimbursement 
e) exit the school by 22:10
f) not wear outdoor footwear in the gym

The member also agrees to indemnify and save harmless:

a) the Executive Committee of the Hillcrest Badminton Club and its members, 
b) the Ottawa Board of Education and its officers, trustees employees and
contractors from all claims, costs and liabilities which may arise as a
result of his or her membership in the Hillcrest Badminton Club or his
or her use of Club facilities at Hillcrest High School.
