2011/2012 MEMBERSHIP AGREEMENT

Memberships are not transferable unless approved by the Club Executive.
Forms must be signed by the individual applying for membership. Unsigned
forms will NOT be accepted. Food and drink may not be taken into the
gymnasium. Only soft-soled, non-marking shoes not worn outdoors may be worn 
inside the gym.

As a player/member of Hillcrest Badminton Club, I agree to the following:

a) I play at my own risk

b) I am responsible for my own acts and omissions

c) I am responsible for the loss of or damage to personal property 

d) I must conduct myself responsibly at all times or my right to 
play may be revoked without notice or reimbursement. 

As a player/member, I also agree to indemnify and save harmless:

a) the Executive Committee of the Hillcrest Badminton Club and its members

b) the Ottawa Board of Education and its officers, trustees employees and
contractors from all claims, costs and liabilities which may arise as a
result of his or her membership in the Hillcrest Badminton Club or his
or her use of Club facilities at Hillcrest High School.
